Unintended acceleration has come back to haunt Toyota once again. The OEM is expanding an earlier recall from 2009 involving Lexus models with floor mats that could entrap the accelerator pedal. It is now adding 131,800 RX350 units and 22,200 RX450h vehicles, both from the 2010 model year.
